Model Application Form

I acknowledge and agree that as a model for Boon Aesthetics Training Academy, I grant permission for any photos or videos taken of me before, during or after procedures and/or aesthetics services to be used for educational and promotional purposes. This includes print, visual and electronic media for teaching, training, marketing and advertising, including but not limited to our website, social media and printed collateral. I acknowledge that I will not receive compensation for the use of these media assets and that they may be modified, edited or combined with other materials as needed.
